Primary Care Update

We are not currently accepting new primary care patients.

Effective July 1, 2026. Established PrimeCare primary care patients may continue to schedule with their provider.

New Patient Requests

Are you considered a new primary care patient?

PrimeCare considers you a new primary care patient if any of the following apply.

You have never been seen by PrimeCare.

You have not been seen by PrimeCare Primary Care within the past three years.

You have been seen by PrimeCare Urgent Care within the past three years but have not established with Primary Care.

Please note: submitting a new patient request does not mean an appointment has been scheduled. Requests must first be reviewed by PrimeCare staff.

Established Patients

Already established with Primary Care?

Use LEAP to access eligible scheduling options with your PrimeCare provider.

Open LEAP Sign in to your patient account.

Find Care Select your provider and eligible visit type.

Confirm your visit Follow the prompts to complete scheduling.
New Patient Requests

Requesting Primary Care for the first time?

If new patient scheduling becomes available, requests are submitted through LEAP and reviewed before confirmation.

Create your LEAP account Patients without an existing PrimeCare chart should begin by registering.

Select Find Care Choose the appropriate provider.

Select NEW PRIMARY CARE Complete the requested information.

Watch for confirmation Staff will review the request before the appointment is confirmed.

Each patient needs a unique email address.

The email address used for your PrimeCare chart must be unique to you and cannot already be associated with a spouse, child, parent, or another patient’s account.

Full legal name and date of birth
Current address and phone number
Your unique email address
Current insurance information
Reason for requesting an appointment
Current medications
Relevant medical history
Recent hospital, ER, specialist, lab, or imaging records

01 / INSURANCE

Verify your insurance.

Please confirm that PrimeCare and the provider you intend to see participate with your insurance plan. Participation may vary by product, network, and provider. Visit our insurance information page for additional details.

02 / MEDICAL RECORDS

Arrange prior records.

If you are transferring care from another practice, please arrange for relevant prior medical records to be sent to PrimeCare. Visit our Medical Records page for instructions.
